5.2 Underwriting Process
The application is the initial source of information to get the process started. The applicant completes the form and the agent does his/her best to make sure it is filled out completely and accurately. Each application must be signed by the proposed insured, the policyowner (if different from the insured), and the agent. In the case of a firm or corporation, usually one or more partners or officers generally must sign the application.
In Florida, a child must be at least 15 years of age to sign a life insurance application; otherwise, an adult (i.e., parent or legal guardian), must sign for and on the minor's behalf.
